Wellness Therapy Initial Consultation $175

A personalized clinical consultation to review your health history, wellness goals, medications, and individual needs. Your provider will determine appropriate wellness therapy options and create a personalized treatment plan.

The $175 consultation fee is applied toward your first wellness treatment after your personalized plan is established.

Follow-Up Visit  $125

In person or video visit for establlished patients for routine, ongoing care, medication management, or addressing changes in your health.

Complex Forms   $85

Everything above plus in-office 12-lead ECG with interpretation and report when clinically indicated

• • Complex Preoperative Evaluation: $325

Extended record review, multiple comorbidities, risk calculation, complex medication planning, EKG and coordination with specialists

• • Required clinical re-evaluation: $115

New visit required due to abnormal labs, uncontrolled condition, medication adjustment or a material change in health
