Scheduling a pre-operative physical does not guarantee that all testing or documentation can be completed before a surgery date that is very close to the appointment.

Review of the forms, testing instructions, and documentation requested by your surgeon or procedural facility.
Laboratory testing, EKG, or other studies may be arranged when required by the surgeon or clinically indicated.
A medical assessment addressing your current health status and readiness for the planned procedure.
Completed evaluation documentation and available results may be sent to your surgeon with your authorization.

Surgeon’s name, telephone number, and fax number
Date and type of procedure
Current medication and supplement list
Relevant specialist records
Recent laboratory, imaging, EKG, or cardiac results
Any forms requiring completion
